Welcome to the Humana Physicians Settlement ("Settlement" or "Proposed Settlement") website established in connection with In re: Managed Care Litigation MDL No. 1334.

This website is intended to provide information to Class Members about the Settlement with Humana Inc., which also involves current and former direct and indirect subsidiaries and affiliates of Humana Inc., in the class action lawsuits known as In re Managed Care Litigation which include Shane v. Humana, et. al, known as the Shane I and Shane II actions and collectively referred to as the "Actions". For a complete list of Humana affiliates, please refer to pages 1 and Exhibit 1 of the "Notice of Proposed Settlement of Class Actions" ("Notice"), which was mailed to Class Members on November 18, 2005 and which is included on this website. As used on this website, capitalized terms have the meaning ascribed to them in the Settlement Agreement.

In order to submit a claim and obtain compensation under the Settlement, you must have provided Covered Services to members of health plans insured or administered by Humana, its subsidiaries or affiliates. For a complete list of Humana affiliates and subsidiaries, please refer to Exhibit 1.

As described more fully in the Question and Answer ("Q & A") section of this website, Class Members were mailed Notice explaining their rights under the Settlement as well as a Claim Form and Claim Form Instructions on how to fill out the Form. This website includes those mailed documents sent to Class Members. Accordingly, Class Members should check for updates to this website to ensure that they have received all relevant instructions.
